Instructions regarding your planned hospital admission
- Please ensure that you have completed the Hospital Admission Form by:
- Completing the ‘Patient to Complete’ sections on the form.
- Completing the Patient Consent (dating, signing and having your signature witnessed where appropriate).
- Lodging the Admission Form with the hospital as soon as possible. At least 1 week prior to your procedure.
- (Allow 15 minutes to do this as the hospital staff will go over the form with you)

If you have not heard from the hospital by 4pm the working day prior to your procedure, please contact the relevant hospital on the numbers above.
Fasting Instructions
- Morning Operation:
- Please fast from midnight the previous day. You must have nothing to eat or drink when you get up in the morning.
- Afternoon Operation:
- You may have a light breakfast (cereals, tea and toast) on the day of your operation. The pre-admission staff from the hospital will inform you what time to start fasting e.g. 7:30am. You must have nothing to eat or drink from this time.
